Anthem Blue Cross Blue Shield usually pays the subscriber (you) directly, there are happy exceptions such as Google and some law offices, which pay the Out of Network provider directly because they have negotiated this with Anthem.
Aetna takes about two weeks to process, Meritain six weeks, Cigna three weeks, United three weeks, and Anthem BCBS four weeks to process. Oxford does not usually cover acupuncture or manual therapy for OON Acupuncturists (unless you have an unusually expensive policy with Oxford), since the higher you pay into your Group policy, the more the Group pays out. But I will nonetheless send them a claim.
