For new patients - please arrive on time, during the initial visit there is 30 minutes Evaluation, as well as, mandatory arbitration consent paperwork to be filled out. Acupuncture Initial 75 -85 minute Evaluation with Treatment visit: I require a patient responsibility direct payment co-insurance deposit of $160 at the time of service. I will send a claim in to your insurance company for the services rendered during the initial 75 to 85 minute visit at the established fees for each service rendered, usually four modalities. The norm is Evaluation 30 minutes, acupuncture 30 minutes, manual therapy 15 minutes or longer. depending on your Commercial Group Plan.
After a few sessions, we will know how your Group Plan pays for each modality. Only then will I know the specific percentage the patient must pay the provider depending on whether their policy is 60-40%, 70-30%, or 80-20% For example, the insurance company pays 60% of the services billed and the patient pays 40% This is called fair and equal billing.
Follow-up sessions: The patient pays the established co-insurance amount or the $160 hourly rate. After each session, I will send an electronic claim directly to your insurance company for the service codes rendered during the visit.
After the first few sessions, we are able to determine the co-insurance payment amount.
I use Venmo or CHASE Zelle to reimburse you any over-payment amount as soon as I receive the (EOB) Explanation of Benefits and the check from your health insurance company; Aetna takes two weeks to process, Cigna three weeks, United and BCBS four weeks to process. Oxford does not cover acupuncture for Out of Network providers (unless you have the Friendship PPO policy).
